We hope the following documents will answer some questions you may have regarding the donation request process. The Referral Flow Chart shows the flow of a routine referral of organ and tissue donation as well as the process for notification of all deaths. The attached Tissue Criteria sheet may change without notice; therefore, it is always good practice to contact the Mid-America Transplant Services Communication Center to pre-screen prior to approaching families. A Request for Anatomical Gifts (under the Uniform Anatomical Gift Act) must be completed on all approaches for donation. Attached is a Sample Consent Form. |
